Please plan to arrive 10 minutes prior to your appointed time. This allows time to copy your driver’s license, insurance cards, make certain records are in order, or fill out necessary paperwork if that has not been completed. Dr. Fritz makes every attempt to be prompt because he respects your time and his ! Your wait will be minimal. In the event your appointment must be canceled, a 24-hour notice is required and appreciated. We do not “double book” appointments.

In consideration of those patients who are sensitive to perfumes, colognes and lotions, we ask that you refrain from using these while visiting the office.

The day of your appointment, please bring with you any of the medications you are currently taking. Supplements and vitamins should also be included. Please do not list these, but do bring in the actual bottles.

To all Patients:
Payment is expected at the time of service for all charges. We will file your claim to your insurance company. If coverage is available under your particular policy, reimbursement will be sent directly to you. We send our patients to Southern Diagnostics Laboratories for required lab testing. The major insurance providers are honored.

Your first visit with Dr. Fritz may take up to an hour. Please plan for spending some quality time for this important visit. This initial visit/consult fee is $250.00. Follow up visits with Dr. Fritz are $100.00 each. Dr. Fritz is a board certified Family Practice physician that operates as an “out of network” provider for all major insurance companies.

To all Medicare patients:
Dr. Fritz is not a Medicare provider. You will have to sign the Private Contract, which states that you will not submit a claim to Medicare or ask Dr. Fritz to submit a claim on your behalf. Any service that is performed cannot be filed to Medicare or any supplemental insurance company. Lab testing is honored at Southern Diagnostics under Medicare.
